If you have made an appointment, our Patient History forms are available to download.
If your appointment is with Dr. Dallas Hancock: Hancock Holistic Clinic
If your appointment is with Flo Barber-Hancock: NICS Therapy Center
If you have appointments with both, ONLY do the forms for Dr. Hancock.

If you are bringing a child under 18 years of age, or a Dependent age 18 or
older, to see either Dr. Hancock or Flo Barber-Hancock, please use this
Dependent History Form

Please complete them at home and bring them with you to your appointment. Thanks !
